FREEDOM OF INFORMATION REQUEST REFERENCE NO: 474-2025
Further to your request FOI-261-2025, responded to as follows:
1) The number of deaths of women referred by the force to be considered for domestic homicide reviews/domestic abuse related death reviews between 2019-2024
Q1 response: Please note that records commenced in March 2020 and from that time to 31st December 2024 there were 22 referrals made.
2) How many of the referrals identified in response to question 1 resulted in the review process being undertaken and how many did not
Q2 response: 17 went to DHR/DARDR in Warwickshire, 2 were adopted by other CSPs outside of Warwickshire and 3 did not meet the DHR criteria.
3) For each death identified in response to question 1, please provide the number of DASH assessments completed by the force in relation to the deceased prior to their death and the risk level identified in each assessment
Q3 response: Please note that we moved from DASH to DARA (Domestic Abuse Risk Assessment) in October 2022. 66 DA risk assessments (DASH/DARA) were completed in relation to those individuals, broken down as follows:
Standard Risk – 9
Medium Risk – 44
High – 13
4) For each death identified in response to question 1, please provide the number of occasions on which the deceased was discussed at the Multi-Agency Risk Assessment Conference/High Risk Daily Meeting/Daily Risk Assessment Management Meeting or equivalent meeting prior to their death.
Q4 response: Of the individuals included in the response to Q1, 5 were discussed at MARAC 13 times (one of those was in another force area).
We cannot answer if the individuals ever had risk assessments or were heard at MARAC in another force area unless stated.
