    • Failed to follow the Warwickshire Police Domestic Violence and/or the Crime Recording Policy.
    • Failed to properly record the incident; and/or
    • Failed to properly investigate the incident; and/or
    • Failed to properly manage the incident; and/or
    • Failed to take positive action and/or reasonable steps to protect Colette Lynch.
